Electronic W-2 and 1095C Information

W-2s are available to view and print via Portal. For those that did not elect electronic consent, your W-2 will be mailed by January 31st to your permanent address on file. Please find instructions below on how to print your W-2.

Instructions to access your electronic Form W-2
• Log in to your UNA Portal
• On the Employee Resources Card select the Employee Dashboard
• Under Pay Information, click Taxes
• Click W-2 Wage and Tax Statement
• Select Tax Year
• Click Display
• Click Printable W-2 – If prompted, your PIN is your 8-digit birthdate (MMDDYYYY)
• For a larger W-2, change your printing options to landscape before printing

Yearly benefit statements required by the “Truth in Salary Act” were sent out on Tuesday, January 20.

Also, in the coming week you will receive an email like the screen shot below from our 1095C vendor. Yes, this email is legitimate. The document is sent from our vendor each year to all that have health/dental insurance coverage for tax preparation purposes. You will receive an email if you had health insurance at any point in 2025 through UNA as the primary person on the insurance coverage.
